Mechanism

SS-31 (elamipretide) is a tetrapeptide that concentrates in the inner mitochondrial membrane and binds cardiolipin, stabilizing cristae structure, reducing oxidative stress, and supporting ATP production. MOTS-c is a mitochondrial-derived peptide that activates AMPK metabolic signaling and influences glucose and lipid handling. SS-31 itself gained a narrow FDA approval (Forzinity, 2025) for Barth syndrome, but MOTS-c remains a research compound, and there is no controlled human efficacy or safety data for this specific combination — the blend is sold grey-market, not as the approved indication.
